Horizon Eye Care is a participating provider for a number of insurance plans, but in the event we are an out-of-network provider with your insurance carrier you may still be eligible for benefits.  Begin by contacting your Human Resource Officer or your insurance company directly and ask them about your out-of-network benefits.  Many cover more than exams and services.  They may cover glasses and even contact lenses.

Once you know what your benefits are you can file for reimbursement.  Horizon can provide you with a receipt or an insurance form which can be sent to your insurance company so that they can reimburse you directly.  You may also have funds available in your HSA (Health Savings Account) or FSA (Flex Spending Account) that can be used to pay for out-of-network services and materials as well.

Our routine eye exam fee is priced competitively at $155 in 2009 (not including any contact lens fitting fees).  Some insurance companies may provide you a benefit of $50 or $60 for example.  You would pay Horizon Eye Care $155 at the time of service for the routine eye exam and then file a claim with your insurance carrier for reimbursement.
